PAs are found in some, but not all, comfrey specimens. David Hoffman in his excellent book Medical Herbalism mentioned four studies that looked for PAs in dried comfrey and none of them ever found any. PAs tend to be more prevalent in the root than in the leaf. They also tend to be more prevalent in the young leaves than in the older, mature leaves. They tend to be a little higher in Russian comfrey than in regular comfrey.
One could, perhaps, reduce the risk of exposure to PAs by using older, mature leaves rather than young leaves or roots. I think we can also say that if one wanted to minimize the risk, one could take some liver- supporting herbs, such as milk thistle, along with the comfrey. I would not ever recommend it be taken internally at high doses or for prolonged periods of time. Don’t use it internally in pregnant women or nursing mothers. Don’t use it in people with liver issues. Don’t use it in people taking pharmaceuticals.
All that said, for it’s tissue-healing properties comfrey generally works as well topically as it does internally. For it’s demulcent effects, it could be easily replaced with marshmallow (Althea officinalis).
